MGM Visa Card
– New cards should be mailed the second week of September. Please check your mail. If you need to file a claim before
receiving the new card, then go to benefits web address at www.wtxebc.com,
and print a Medical Reimbursement Claim form located under Benefits and
Forms. Guardian Dental Cards
– Beginning last year, Guardian started mailing dental cards to the
participants’ home address. If you have lost your dental card, then go
to the benefits web address www.wtxebc.com
and print off a dental card (located under Benefits and Forms and Dental).
New hires that have elected dental will receive a card from Guardian. Vision Cards
– Superior Vision has mailed cards to participants’ home addresses. If
you have lost your card or if you don’t receive a card through mail, then
contact FBS to request a new one. |
